Summary Description: The Dietitian provides services and education to residents and staff of SNF to support and maximize the patient's nutritional well-being. This position reports to the Facility Administrator.Come in 6-12 Hours a Week, Flexible Hours*RESPONSIBILITIES:*1.) Evaluates, develops and completes comprehensive nutrition assessment and reassessment consistent with CMS regulations.2.) Works with patient and IDT to establish, document and update plan of care (POC) to achieve short- and long-term goals.3.) Assesses on-going nutritional status to determine appropriate nutrition intervention(s) and recommendations. Consults physician regarding lab tests as needed.4.) Identifies and develops the treatment and nutritional goals for POC.5.) Provides nutrition education and counseling based on individualized patient needs.6.) Selects or develops education materials and methods appropriate for individual patient.7.) Documents nutrition progress upon admission, quarterly, and with significant change of condition.8.) Collaborates with Dietary Manager to facilitate adequate and appropriate nutritional care.9.) Reviews menus quarterly with Dietary manager, addressing nutritional content, palatability, and presentation.10.) Reviews residents who have wounds or significant weight loss for nutritional needs.11.) Consults with Nursing on the nutritional needs of residents with eternal feeding tubes.12.) Recommends improvement initiatives and uses analytical and critical thinking skills to implement innovative solutions to problems. Provides weight loss report to QAPI meeting.13.)
