foster care for children with disabilities. Summary: As a member of
the interdisciplinary team, the Clinical Therapist is responsible
for providing trauma-informed therapy services to individuals with
mental illness, who also have a secondary diagnosis of intellectual
or developmental disabilities (I/DD). Clients may also present with
various physical challenges. The role will involve client
engagement, therapy, treatment planning, and collaboration with
internal and external stakeholders.
Essential Responsibilities for the Peer Mentor, include but are not
limited to:
- Provide outreach and engagement services to individual clients
participating in the ACT Program
- Share stories of own success in accordance with comfort
level
- Facilitate group conversation with clients, family members and
legal guardians about available resources at Envision Unlimited and
the MH system at large.
- Have input in the development of treatment plan goals
- Accompany clients on recreational activities and Drop-in Center
events
- Assist staff to develop resources for clients and link them to
appropriate services
- Provide individual and group peer counseling
- Able to Develop Crisis and medication adherence plans with
clients Required Qualifications for Job Execution:
- Community Presence: Must be willing to travel and engage with
clients in the community 50-75% of the time.
- Transportation Requirements: Must have a valid driver's
license, a reliable vehicle, and active car insurance to meet
community service needs.
- Work Hours: Day Shift, Monday to Friday (CST).POSITIONS
SUPERVISED: No supervisory responsibilities. Essential
Responsibilities:
- Manage New Client Referrals: Collaborate with Day Center staff
and Intake Coordinator to manage new client referrals, including
obtaining consents, communicating with guardians, and liaising with
site staff.
- Conduct Mental Health Assessments: Utilize the State-approved
IM/CANS tool for comprehensive assessments and semi-annual
treatment planning for assigned clients. Ensure treatment plans
incorporate client-specific needs and objectives.
- Provide Individual Therapy and Assessment: Deliver one-on-one
therapy, individual assessments, and treatment planning for dually
diagnosed clients (mental illness with I/DD) ages 18-65+
years.
- Therapy Services Delivery: Provide therapy services both
in-person at EU day centers on the Southside of Chicago and via
telehealth at the Ravenwood location, as per client preference and
treatment needs.
- Collaborate with External Agencies: Coordinate with outside
agencies to ensure appropriate service provision and continuity of
care.
- Communication and Coordination: In collaboration with the
client's treatment team, keep families, guardians, and other
Envision Unlimited staff informed about client progress and
treatment outcomes.
- Clinical Documentation: Maintain compliance with all clinical
documentation standards for assigned clients, ensuring timely
completion and adherence to productivity targets.

Requirements:EDUCATION, SKILLS, and/or EXPERIENCE:
- Master's degree in Social Work, Counseling, Psychology, or a
related field.
- A minimum of three years' experience in social services, with a
focus on mental health or intellectual and developmental
disabilities (I/DD).
- Experience working with diverse populations with multi-faceted
needs, including mental health and I/DD populations.
- Knowledge of Medicaid standards is preferred.
- Strong written and verbal communication skills.
- Excellent decision-making, problem-solving, computer, and
organizational skills.
- Ability to work autonomously with minimal supervision.
- Must possess a valid driver's license, a reliable vehicle, and
